What is the Medication Therapy Management (MTM) program?

According to the Centers of Medicare and Medicaid Services, Medicare Part D plans must provide to their beneficiaries a free in-depth one-on one review of the beneficiary’s medication profile. If you have a Part D prescription plan, you may qualify for a Medication Therapy Management, or MTM that is provided by pharmacists at all Nash Drugs locations.

Your Medicare Part D plan determines eligibility by looking at your medication claims information every month. If you have at least three of the five following conditions like chronic heart failure (CHF), hypertension (high blood pressure), high cholesterol (dyslipidemia), osteoporosis, or diabetes, are taking eight or more medications in a 30 day period and are likely to incur annual Part D medication costs of at least $3,138, your Part D plan will notify you and us that you qualify.

After receiving a notification, our pharmacist will contact you to schedule a time to review the medications you are currently taking. This review is to help answer your questions you may have about your drug therapy, discuss side effects or concerns you may be experiencing, review your medications for alternatives that may help to lower drug costs and help you with ways to keep track of refilling and taking your medications.
